ABSTRACT

Treatment of pain related to various types of deafferentation remains a neurosurgical challenge. Medical therapy and conventional surgical techniques for pain relief have not been effective due to this problem. Six patients with intractable pain following a brachial plexus injury were treated with dorsal root entry zone(DREZ) lesions. These lesions were made with CO2 laser. All patients were followed from 5 months to 5 years after surgery. Two-third of the patients were relieved of more than 50% of their preoperative pain. Post-operative neurosurgical complications were ipsilateral leg weakness and loss of proprioception in 2 cases. The laser technique is exact, makes uniform lesion, shortens the duration of the procedure, lessens cord manipulation and makes shalow penetration into the surrounding spinal cord. Dorsal root entry zone(DREZ) lesions made with CO2 laser appeared to be a satisfactory treatment for brachial plexus injury patients who have failed to respond to more conservative modes of therapy.

ABSTRACT

The composition of triglycerides of the axillary sebaceous glands in 10 patient, with osmidrosis and 10 patients with hyperhidrosis was analyzed by high pressure liquid chromatography. Sebaceous glands were isolated and dissected by Kellum's method, and lipids were extracted by Folchs method. Individual triglycerides were separated by high pressure liquid chromatography and fatty acid composition of triglycerides was analyzed by gas-liquid chromatography. A number of individual triglycerides were identified: Fraction 1 consisted of dimyristoarachidonin, dilinoleioarachidonin, and myristol inoleioarachidonin, fraction 2 was trilinolein, fraction 3 was dilinoleioolein, fraction 4 was dilinoleiopalmitin, fraction 5 consisted of dioleiolinolein, dipalmitolinoleiri, and palmitooleilinolein. In osmidrosis patients, cornpositions of fraction 1, 2, 3, 4, and 5 were 5, 4%. 18.5% 29.5% 32.0% 14.1% of total triglycerides, respectively. In hyperhidrosis patients, cornpositions of fraction 1, 2, '3, 4, and 5 were,5, 9%, 20. 6%, 30. 9% 32. 1%, 14. 5% of total triglycericles, resectively. There were no differences in composition of triglycerides of sekaceous glands between osmidrosis patients and hyperhidrosis patients and hyperhi.frosis patients.

ABSTRACT

We have experienced 3 cases of localized cervical adhesive arachnoiditis. 2 of them had history of operation under spinal anesthesia. Paresthetic pain and weakness are the commonest presenting symptoms and signs in our cases. Myelographically, findings are simulating the intramedullary lesion in 2 cases. With surgical intervention, 2 cases have good results.

ABSTRACT

Of the case of malignant astrocytoma, spontaneous transdural extension is very rare. Only several cases of transdural extension of primary intracranial tumor are reported. However, these cases are through the foramina of the skull base. We have experienced a case of malignant astrocytoma which directly extended out through the dura and calvarium near the pterion of the left side.

ABSTRACT

The patient is a 17 month old girl admitted to the Korea General Hospital with histories of irritability, walking disturbance and right sided weakness. On admission, she had seizure followed by coma. Skull x-ray showed widely separated all cranial sutures. A left carotid angiogram revealed a space occupying lesion in the left frontotemporoparietal region with shifting of the midline to the right. At operation, a darkish hemorrhagic tumor with relative clear demarcation was explored after evacuation of massive intracerebral hematoma. The tumor was extended to the midline. The main mass was removed with difficulty. Additional fragments of the necrotic tumor tissue infiltrated in the surrounding structures were also removed. It was not directly connected to or have invaded the ventricular wall. On microscopic examination, the tumor was largely made up of solid sheets and cords of round or ovoid cells with darkly stained nuclei. In areas, typical ependymal rosettes as well as pseudorosettes, gliomatous features and occasional formation of tubule are presented. We think the tumor represents those rare tumors called ependymoblastoma of Rubinstein.
